Assessment Scheduling Day

All the Spring 2024 semester appointments have been filled. Psychological assessments for the Fall 2024 semester will begin scheduling on Monday, August 5th. On that day, starting at 9 am, please visit our online portal to request an assessment. Instructions for requesting an appointment:

  1. Visit the online portal
  2. Click the box labeled “assessment request form”
  3. Fill out client information. Be as detailed as possible. If you are requesting an appointment for someone other than yourself (like a child), please use THEIR name in the “client name” portion.

PLEASE NOTE: the appointment request form will not be available until Monday 8/5 at 9am.


Thank you for providing information about your appointment request process. To ensure that your request is processed correctly and efficiently, please follow these guidelines:

  1. Submission Order: Appointment requests will be processed in the order in which they are received. It's important to submit your request as soon as possible to secure your place in the queue.
  2. Assessment Type: Consider specifying the type of assessment you are requesting. Different assessments may require specific expertise from student clinicians and supervisors.
  3. Relevant Details: In your submission, include any relevant information about the case. This may include previous diagnoses, the name of the referring provider, and the reason for the diagnosis. The more information you provide, the better equipped the office will be to assess your request.
  4. Limited Availability: Be aware that space for appointments is limited. Unfortunately, not all requests can be accommodated due to this limitation.
  5. Referrals: If your request cannot be accommodated, the office may offer referrals to local area providers who offer similar services. This can be a valuable resource to consider if you are in need of assistance.
  6. Status Update: The office will contact you by the end of the week to provide the status of your appointment request. Be sure to provide accurate contact information to ensure you receive this update.

Following these guidelines will help streamline the appointment request process and increase the likelihood of your request being accommodated or receiving valuable referrals if needed.
