Forms for Academic Actions

The forms below must be completed by certain deadlines during the academic year. Please consult the Academic Calendar (see link at left under Resources) for the appropriate deadlines. See detailed instructions for each form; some forms are available online and some are available for pickup in the Advising and Academic Support office, Room 323, Hall of Languages. Click on the name of the form to print a form or view a sample accordingly.


FORM PROCESS
Add/drop

1) Pick up form from 323 HL. Fill in 5 digit class number, course prefix, 3 digit course number and number of credits

2) Gather signature from Instructor Or Department Chair (note: for Math courses the signature must be from the instructor except on the day of the Drop Deadline. On this day only, you may obtain a signature from the Department Chairperson in the Math Dept.)

3) Sign and submit form to 323 HL for Home College Signature (If your home college is Arts & Sciences)

4) Deliver form to 106 Steele Hall (Registrar's Office)

Withdraw Petition

1) Print and complete 2 forms

2) Gather signatures on both forms from Department Chair or Instructor

3) Sign and submit 2 forms to 323HL for Home College Approval

4) Deliver 1 completed form to 106 Steele Hall (Registrar’s Office)

Declaration of Major

1) Print and complete 1 form

2) Gather signature from Department Chair

3) Sign and submit form to 323 HL for Home College Signature

Declaration of Minor

1) Print and complete 1 form

2) Gather signature from Department Chair

3) Sign and submit form to 323 HL for Home College Signature

Petition for B.S.

1) Pick up form from 323 HL

2) Complete form/gather signatures from Department Chair AND Faculty Advisor

3) Sign and submit form to 323 HL for Home College Signature

Transfer Credit Petition 
(Petition completed and approved
prior to taking the course at another institution)

1) Print and complete 1 form

2) Gather signature from Department Chair (only if petitioning for major/minor, language, math or writing credit)

3) Include a course description AND syllabus

4) Sign and submit form and description AND syllabus to 323 HL for Home College Signature (You can also fax to 315-443-9397 or email to casadvising@syr.edu)


PLEASE NOTE: If you are taking transfer credit at another Institution in order to complete your degree, all coursework (including final exams) must be completed by December 21, 2012, May 12, 2013 or August 23, 2013 in order to be certified for December 2012, May 2013 or August 2013 respectively. Work completed after the above dates automatically changes your graduation date to the next graduation cycle.  The College of Arts and Sciences will use the academic calendar of the institution where you plan to take transfer credit and will consider their last day of final exams to be the last day of classes for purposes of determining when coursework was completed.

Internship Proposal/Agreement

1) Print and complete 1 form

3) Sign and submit form to 323 HL for Home College Signature

Pass/Fail Application

1) Pick up form from 323 HL

2) Complete, sign and submit form to 323 HL for Home College Signature

3) Deliver form to 106 Steele Hall (Registrar's Office)

Audit Application

1) Pick up form from 323 HL and complete

2) Gather signature from Instructor

2) Sign and submit form to 323 HL for Home College Signature

3) Deliver form to 106 Steele Hall (Registrar's Office)

Request for Incomplete Grade

1) Print and complete 1 form

2) Gather signatures from Instructor AND Department Chair

3) Sign and submit form to 323 HL for Home College Signature

Leave of Absence/University Withdraw

1) Come to 323 HL to meet with an academic counselor

2) Complete, sign and submit form to 323 HL for Home College Signature

3) Deliver form to 306 Steele Hall (Office of Student Assistance)

Application for Readmission

1) Come to 323 HL to meet with an academic counselor

2) Complete, sign and submit form to 323 HL for Home College Signature

3) Deliver form to 306 Steele Hall (Office of Student Assistance)

Intra-University Transfer Application

1) Attend a mandatory I.U.T. meeting to receive application

2) Complete forms and return to 323 HL by semester deadline

Petition for Early Participation in Commencement

1) Pick up form from 323 HL

2) Complete, sign and submit form to 323 HL for Home College Signature

Petition to Faculty (Blank Petition)
